Reality only exists as long as we are conscious of it.  The world may
not vanish for others if I lose awareness of it, but since this is MY
view we are talking about, then it does vanish for me if I lose
awareness of it.  Since all reality is filtered through this
consciousness, then what this consciousness perceives IS reality. 
Even knowing that reality goes beyond this consciousness, the
knowledge that it does is still filtered through it, hence it is still
a subjective experience.

What if I'm surrounded by pink dinosaurs?  I'm not, because I'm not
conscious of it.  It doesn't matter if someone can prove that I am, as
long as I am not conscious of this fact, then this is reality for me.

In short, reality is subjective.  While there may be an objective
reality in theory, in actuality it's all subjective.  Even learning of
"objective" reality is a subjective experience.

"That we are conscious of" is an object of consciousness, not
consciousness itself.  A red car is not consciousness but is something
that I am conscious of.  Consciousness is awareness.

I'm not sure what happens.  We may lose consciousness of our self, or
we may be aware of it but we may not be moved by it.  You can reach a
state where you see your sense of self as just another sensation, like
an itch or a scratch.  In this case, you are conscious of yourself,
but you have an entirely different view of it.

As far as I know, it continues.  The difference is in how you view
things, not that you cease to view things.
