That's merely a result of people demanding that their employers provide
medical insurance for non-catostrophic illness and absorb most of the cost
of that insurance for the past few decades. 

Since the ultimate consumer didn't see the true cost of the insurance, nor
the true cost of their medical care, they really didn't care all that much
as costs climbed. 

Now that some people are being forced to pay for what they used to get
"free", they're shocked at what they've done.
