This was a movie I saw last night, in which a quadripligic wanted a switch installed which empowered him to cut of the respirator and let him die. He won an injunction in the Georgia court, to have that switch installed, so that he could die if/when he wants. He eventually decided to go on living. But it was pretty obvoius that he valued the freedom to die when he wants. In most cases, the disabled person has no choice. If he/she keeps going on about how much he/she wants to die, what will happen? The choice is simple: Resign yourself to having to live, and your family/caregivers will give you some things that'll make life more bearable. Stay angry and keep going on about how much you want to die, and people will get tired of you -- which will, of course, make a difference in their interest in your comfort.
