Medical Codes = Bad Medical Care?

The doctors & nurse-practitioners say, that the use of medical codes
(used by the govt & private insurance companies), ... is barring them
from ordering the testing & medical care that the patients need & want,
.. such as better bloodwork [http://www.gsdl.com].

What can patients do, to help override this very sick & sleazy monopoly
which some powerful groups (like AMA, Medicare, etc) now have, over our
medical system in the US?

What is the process for getting more & better codes through? And for
helping to cancel the worst tests, ... such as the use of EMG/NCV &
CAT-scans to test for most spine injuries, ... when these tests
repeatedly show nothing? [Also, these tests are being used by the worst
doctors, to justify their non-diagnosis & non-treatment of
seriously-injured patients, ... to waste patients' time & resources, &
to run-up high medical bills for no good reason.]

Other testing is so much better & more practical/logical/useful for
spine injuries, ... such as motion x-ray [http://www.nu-best.com] &
stand-up MRIs [http://www.fonar.com].

How can we get some codes for better blood testing, through the now very
failed medical system in the US? What is the process?
