I mention here criminal intent as in murder or manslaughter and you call it
"silly" in this part.

Hypothetical questions require hypothetical answers which are conditional to
the premise "pushes".

So why did you call my response silly above then Marky.

If the only thing you have is an MRI to rely on abuse then you are in
trouble.
A criminal act is an act of intent and intent must be proven.  That is not a
medical problem but a criminal court one.  When a criminal act is suspected
in a health setting then the police are called in.  Very few injuries can be
classified as out right abuse.
A friend of mine had a 6 year old that wasn't watching where he was going
and ran into the door or wall as she wasn't watching so not exactly sure
where it happened but he ended up with a large bump on his cheek.
She took him to a hospital and was verbally abused by the doctor shouting in
a loud voice that it was abuse.  He was the judge and jury and called
protective services.
The boy never said his mom did it. She was taken aback and shocked by such
allegations as they were investigated and found without merit.  She was put
on watch for several years and nothing ever ever happened again.
You need a hidden cam to monitor such situations otherwise it is nearly
impossible to figure out what happened.  That's the only way to get the
mothers of  munchausen syndrome by proxy.
MRI is not the answer.
