Read the fine print in the disclosure statement you sign.  Mine had a 
list of about a dozen people or agencies who could be given the 
information along with a note saying that in addition my medical info 
could be given to anyone not prohibited by law.  It further said that 
while I could ask for a copy of my medical records, I'd have to pay to 
get them and I might not be given all of them.  Then look at all the 
statements that refer to "providers" and "interested parties" and other 
such phrases, none of which are defined in writing.
