Fibrosarcoma is unlikely with lumps that have been present for 20+
years.... with pain there is always the possibility of plantar
fasciitis or partial tears of the plantar fascia and resultant scar
tissue.

On the payment side which was your question - try contacting the state
podiatric medical association in your area and ask for suggestions,
social workers may be of some help, and you may call and ask the
podiatrists in your area their fees.
