Oh, hell, just remembered.  It's the airforce solution you want
not the navy.

Use a throat microphone.  Good enough for a Spitfire or P51 cockpit.

If your present gadget is really a modified stethoscope then
hold the trumpet of the stethoscope a bit to the side of 
and above the Adam's apple, making an airtight seal to the
skin.  People do sound weird with a throat mic.
