Not an engineering book but worth reading.
Medvedev's "Legacy of Chernobyl" had some interesting sections,
particularly with respect to "systemic" failures related to that incident.
Unfortunately I don't have a copy handy.
Maybe you could look at The Man-machine Interface as a tool to improve
Critical Systems Integrity".
