Adding channels?

Is there any point in telling a cable company the size of Comcast or TCI
that you want them to add a channel they don't carry?

Or are their carriage decisions based entirely on private deals between
themselves and the networks, and affecting all the systems they own (ie.
Cox won't carry Hallmark Channel in East Jesus, Tennessee unless they've
already decided to carry it nationwide on ALL their systems)?
