Even if some of the things on the list are theoretically "legal" according to the rules, it's all about intent. Spirit of the Game comes first, and anybody who pulls that crap knows what they're doing violates the spirit of the rules. One example came up in a game I worked this summer. One guy was rather flagrantly running into the player he was defending to stop him from being able to put up a huck. Offensive player would inevitably call a foul, and he wouldn't contest. (The story was that he typically contested the fouls, which was one of the reasons our presence was requested.) The third time he did it, I warned his captain that if I (or the other observer) spotted him doing it again, we'd issue a yellow card. Oddly enough, he didn't do it again.
