If the lens is in direct sunlight then a hand or dark slide works fine. However, a lens may also need to be shielded when photograhing in bright diffused light. In that situation the light strikes the lens from all sides and a hand or dark slide does little good. People don't buy dedicated lens shades because the idea of using their hands or a dark slide never occurred to them. They spend the money because they understand that a hand or dark slide doesn't always do the job.
