The literal spelling of the chord is simply C E G Bb D# - exactly what
the symbol says.  But as for what notes it might imply you can use in
your voicings or improvisation, there are several possibilities.  The
blues scale is one, the diminished whole tone scale (seventh mode of
melodic minor, aka super locrian, aka altered) is another, as is really
any combination of notes you choose that captures this sound.  Fifth can
be altered or not as you see fit, although most arrangers would probably
use the symbol "alt" instead if they specifically wanted you to alter
the fifth.
