When I used Fender medium picks, before switching to lignum vitae wooden picks, I would punch 3 holes in each pick with a conventional hand paper punch.

It took a few seconds to do, never cracked a medium pick, and produced holes of sufficient size for the fleshy fingertip to bulge through and stabilize the pick. Also, the picks would feel very comfortable with this modification, and they retain the same flex characteristic.

Finally, if someone borrowed a pick, I'd have the amusement of hearing them say "What the hell is this?" which, incidentally, I am still enjoying with my wooden picks.
