   Since this was not emergency surgery why didn't you check with your
insurer prior to the procedure? From your post it sounds as if you asked
everyone else.
   Asking the doctors' office what your insurer covers is a common mistake.
I can only offer sympathy and suggest that in the future you call the
insurer. Many have folks who answer such questions and "pre-approve"
surgical and medical procedures.
