I'm not a doctor, but given Josh's age and the pattern you've described (fever, followed by a non-itchy rash that spreads), roseola (a virus) came to mind. No doubt many other things could cause the same symptoms (and it's probable your physician considered roseola, which, common as it is, shouldn't be "strange" to a pediatrician).
