From back when the process was developed quickly and scrapping 5% of the output after careful inspection was the norm during and shortly after WWII. A response to the need to get millions of war items out the door as quickly as possible. Then the fabulous move to statistical process control as demonstrated as the prime cause for Japan's sudden dominance in the world of quality. To the today's over emphasis on documentation, ISO standards and so much stuff that adds not one cent of value to the effort... I used your post as an example of just how this ISO system is hampering progress by putting so much into the quality process and so little in the manufacturing process. Today's QA seems to belong to the consultants and not the manufacturing engineers. If the money spent on all audits and ISO paperwork were put into better machines and systems, the world would be much better off. Who needs an inspection and quality system when we can make a process capable of yielding 130% of what we want?
