I think that is the answer: lack of supply, sort of: not enough
competition to drive the prices down. Lack of demand, paradoxically, may
be a cause of this, since the market may not be big enough to attract
enough players.

My guess: that is a problem with private health insurance: whatever is
charged, gets paid. So, prices go up.
