You could check www.internationalsos.com. This company does a lot of
stuff, including some more specialized EMS and air evacuation around
the world. They have a regional office in Caracas Venezuela. They also
have regional offices in Madrid and Barcelona (as far as the
spanish-speaking countries go).

I found them on an ad looking for medics to provide EMS support to the
"civilian contractors" working in Iraq...but they apparently have
operations and potential job opportunities world wide.

Worth checking anyway.
