Yes they do.  Just the other day I saw a show where the medic performs a
conscious cardioversion on an 80 year old lady.  Not sure why he did it as
the FIRST intervention, perhaps their protocol doesn't allow for drug
interventions first.

Anyhow, the woman screams in pain as he welded her, then the voice of the
medic says "this will hurt a little bit."  I'm pretty sure the "this will
hurt a little bit" came prior to the zap.
