Help Pls: CONFIDENTIALITY AND ASSIGNMENT AGREEMENT

Hi

I need some advice pls:

I need to sign a doc titled "CONFIDENTIALITY AND ASSIGNMENT AGREEMENT"
for a Contract position.

I am doing Research for them.  I also would like my name in any
Patents & Publications (I prefer to write).

Is this abnormal?

Secondly, in order to assertain this, I want to attach a piece of
paper and state something before my signature that my signature is
valid with the agreement from the company for the attachment.

Could someone helpe me with some drafts.
